Our Life Coaches develop a deep understanding of the ten residents that live there. We are intentional with staffing; limiting the number of employees in and out of the home to always have a “familiar face” caring for the residents. This cultivates a continuity of care in the home and in turn decreases anxiety for our residents. Our staff, residents, and family members begin to feel like they are part of one big family.
- Care & Safety Checks every 15 minutes
- Programming schedule seven days a week
- Gerontologist/Nurse Practitioner
- Located only 1.3 miles from nearest hospital
Upon admission, a detailed nursing assessment and comprehensive Alzheimer’s/Dementia evaluation is performed, along with medical assessments from the following:
- Activity Director
- Dietician
- Social Worker
- Certified Therapeutic Recreational Specialist
- PT/OT/ST evaluates if applicable
After all evaluations have been performed their individualized plan of care is written and reviewed. Meetings with the responsible party are scheduled on a quarterly basis to ensure open communication is maintained throughout the residents stay.
